high-quality care must go hand in hand with value. This means doctors must meet high quality standards and affordable costs to be included in the Centivo Network.

    The Centivo Network includes local, virtual and national healthcare providers, so you have access to all the services you may need.

    Centivo's primary care-centered plans are created with quality and savings in mind.

    The catch? You need to follow guidelines like choosing a primary care doctor, getting referrals for specialist care and staying in-network.

    You’ve got questions, and we have answers

    • Centivo is a health plan administrator. This means Centivo has contracts with providers (e.g. doctors, hospitals, surgery centers, labs and others) and pays them when you get care. Centivo is a newer healthcare company, so you might not be familiar with the name. However, you are probably familiar with the doctors and hospital systems Centivo works with in your community.

    • Your monthly cost, known as a premium, will vary depending on your plan options and who is covered on your plan. Typically, your premium is taken out of each paycheck.

      If you have a deductible, you’ll pay that amount out-of-pocket before the plan pays towards your healthcare costs. Most Partnership Plans have no deductible, so your plan starts to pay right away.

      When you get care, you’ll pay a copay (a pre-set amount) or coinsurance (a percentage of cost), depending on your specific plan. No matter what, you won’t pay more than your annual out-of-pocket maximum for any covered healthcare and pharmacy expenses during the plan year. Premiums do not count toward your out-of-pocket maximum.
